The Sayville Project’s Health Home Care Management Program has partnered with Community Health Care Collaborative (CCC) of Sun River Healthcare in the New York State Medicaid Health Home Initiative. The Sayville Project currently serves over 400 adults with chronic health and mental health conditions residing on the south shore and in Suffolk County's central communities
The Health Home Care Management Program supports individuals who have active Medicaid and are living with two or more chronic health conditions* or a serious mental health condition. The goal is to help support individuals in managing their chronic health conditions and coordinating their health care needs to maximize positive health outcomes, prevent future hospitalizations, and assist with increasing the independence of our community members. If deemed eligible, individuals are assigned a care manager who will help the member develop personalized care plan goals, support in the coordination of their care, link to community resources, assist with securing and maintaining benefits/entitlements, explore housing resources, and facilitate communication between medical and behavioral health providers.
Individuals eligible for Health Home Care Management Program services must be enrolled in Medicaid and have:
- Two or more chronic conditions (e.g., Substance Use Disorder, Asthma, Diabetes*) OR
- One single qualifying chronic condition:
- HIV/AIDS,
- Serious Mental Illness (SMI) (Adults), or
- Sickle Cell Disease
*Examples of chronic health conditions include, but are not limited to, diabetes, hypertension, asthma, substance use disorder, obesity, HIV/AIDS, and sickle cell disease.
