The Sayville Project’s Specialty Mental Health Care Management Program is an offshoot of the NYS Medicaid Health Home Program, providing intensive support and services to members with a primary psychiatric condition who are at the highest risk — commonly referred to as Health Home Plus (HH+) services.
Overseen by the NYS Office of Mental Health and the NYS Department of Health, HH+ is an intensive Health Home Care Management (HHCM) service established for defined populations with serious mental illness (SMI) who are enrolled in a Health Home.
Individuals eligible for HH+ services must have SMI and satisfy one or more of the following:
- Be on a current or recently expired assisted outpatient treatment (AOT) order.
- Have three (3) or more psychiatric inpatient hospitalizations within the last year.
- Have four (4) or more psychiatric emergency room visits for mental health within the last year.
- Have three (3) or more medical inpatient hospitalizations within the last year and a diagnosis of schizophrenia or bipolar disorder.
- Be currently homeless (HUD 1 category).
- Have been recently incarcerated within the last year.
- Be ineffectively engaged in care.
- Be designated “High Risk” by the local Suffolk County Single Point of Access (SPOA) Unit.
