Specialty Mental Health Care Management Services

The Sayville Project’s Specialty Mental Health Care Management Program is an offshoot of the NYS Medicaid Health Home Program, providing intensive support and services to members with a primary psychiatric condition who are at the highest risk — commonly referred to as Health Home Plus (HH+) services.

Overseen by the NYS Office of Mental Health and the NYS Department of Health, HH+ is an intensive Health Home Care Management (HHCM) service established for defined populations with serious mental illness (SMI) who are enrolled in a Health Home.

Individuals eligible for HH+ services must have SMI and satisfy one or more of the following:

  • Be on a current or recently expired assisted outpatient treatment (AOT) order.
  • Have three (3) or more psychiatric inpatient hospitalizations within the last year.
  • Have four (4) or more psychiatric emergency room visits for mental health within the last year.
  • Have three (3) or more medical inpatient hospitalizations within the last year and a diagnosis of schizophrenia or bipolar disorder.
  • Be currently homeless (HUD 1 category).
  • Have been recently incarcerated within the last year.
  • Be ineffectively engaged in care.
  • Be designated “High Risk” by the local Suffolk County Single Point of Access (SPOA) Unit.
